Human approval is often presented as the answer to agent risk. It is necessary, but a button is not a governance system. Approval only works when the person can understand the action, the scope is clear and the decision can be reconstructed later.

Mature interruption includes revocation, rollback, identity, logging and escalation. It also includes the ability to refuse a task without being punished by an opaque workflow. The human must remain an accountable operator, not a ceremonial click in an automated ceremony.
